• Hold a Bachelor's Degree or Diploma in Health Science Management, Social Services, or a related discipline.
• Have at least 3 years of relevant experience in healthcare or community care operations, including strong people management.
• Fluent in English and at least one additional local language.
• Be proficient in Google Workspace and Microsoft Office applications.
• Willing to travel to visit clients at home as part of care coordination.

Responsibilities

• Lead and manage frontline operations of the assigned zone in Home Care, optimizing manpower and service quality.
• Engage internal and external stakeholders to pilot projects and develop pilots.
• Oversee service coordination: allocate care coordinators in geographic area to ensure timely care.
• Monitor manpower deployment and recruitment/onsite training.
• Conduct assessments and develop care plans, coordinate with nursing team for care standards.
