Position Overview:
Job Bridge Global is excited to announce an opportunity for a full-time RN Case Manager to join our dynamic team. This role is designed for a dedicated nursing professional who is passionate about delivering high-quality patient care and enhancing patient outcomes through effective case management practices.
The Case Manager is responsible for the coordination of care and service delivery for an assigned group of patients and promoting interdependent communication and collaboration with the patient, caregiver, physician, and other members of the health care management team. The Case Manager will assess, implement, coordinate, monitor, and evaluate available services and resources to promote quality, cost-effective outcomes appropriate to patients\' needs and resources.
