Qualifications

• Experience working with young people; autism or learning disabilities advantageous.
• Ability to follow and update each child's care plan to achieve outcomes.
• Understanding safeguarding and child protection principles.

Responsibilities

• Supporting young people within the community, attending clubs, classes and activities.
• Following and updating each child's individual care plan to achieve appropriate outcomes.
• Building positive relationships with young people and helping them reach goals.
• Working with parents and partner agencies to create opportunities for young people.
• Communicating and documenting important information to colleagues and management.
• Attending meetings and contributing to team support and efficiency.
