Kent and Medway NHS and Social Care Partnership Trust is seeking a Healthcare Support Worker to visit young people under 18 in homes, schools or community settings. You will support mental health and emotional wellbeing and help deliver care plans, reporting to the Multi-Disciplinary Team.

The role requires a full UK driving licence and access to a car for business use. You will work closely with families and communities to provide compassionate care in varied environments.

Formación

• Must hold a full, valid UK driving licence
• Interest or experience in supporting young people with mental health needs
• Able to work under pressure and maintain professional boundaries

Responsabilidades

• Visit young people in their homes, schools or community locations to provide support
• Attend daily meetings and safety huddles
• Prioritise allocations and appointments
• Observe and report clinical and non-clinical information on electronic patient records
• Support decision making in the best interests of children, young people and families
