Job summary

Powys Teaching Health Board is seeking a dedicated team member to join the CAMHS Crisis, Home Treatment Service, and outreach support for young people in mental health distress across Powys. The role involves delivering care in clinical settings or community environments and working within a small, dedicated team to prevent crises and support early intervention.

Qualifications

• Experience in CAMHS or mental health settings preferred.
• Ability to work within a multidisciplinary team.
• Commitment to safeguarding and patient-centred care.

Responsibilities

• Work within a multidisciplinary team under the direction of a Registered Nurse.
• Assist assessment by gathering information and contributing to care planning and risk assessments.
• Deliver health care as per the Care and Treatment plan.
• Contribute to group work and activities with a Registered Practitioner.
• Provide 1:1 supervision for patients on the Age-Appropriate Bed.
• Complete referrals to health, public and voluntary services as appropriate.
• Support safeguarding reporting and liaising with schools and care services.
• Contribute to discharge planning and transition support.
