Young People Leaving Care Outreach Support Worker

Position Details

Outreach hours are between 8am and 8pm, Monday to Friday.

This is a long-term temporary position with no specified end date. If you subscribe to the update service and have a short notice period, a quick start through a temp-to-permanent placement is possible.

Locations: Sheffield, Leeds, Hull.

Candidate Requirements
  • Experience supporting young people and their families.
  • Full UK Driving License with Business Insurance (essential).
  • Flexibility to meet the needs of young people, with some flexibility in working hours.
  • Experience in property inspections, supporting independence skills, and managing diaries.
  • Current online DBS check or the ability to obtain one quickly.
  • Experience in similar settings and ideally NVQ Level 3 in Children & Young People Workforce or equivalent.
  • Willingness to undertake further training.
Job Purpose

To provide support for vulnerable young people, working closely with social workers and other agencies to promote independence and well-being.

Main Duties & Responsibilities
  1. Assist young people’s social workers in service delivery, including:
  • Providing transport and escorts as per care plans.
  • Conducting welcome and induction sessions.
  • Assessing and monitoring needs.
  • Supporting welfare and NHS issues.
  • Preparing reports and arranging appointments with partner agencies.
  • Maintaining regular contact with young people and informing them of support groups.
  • Assisting with move-on processes.
  • Maintain accurate electronic records of service users.
  • Attend meetings and participate in team activities.
  • Provide mediation and support as required.
  • Use IT packages effectively to support work.
  • Work cooperatively across multidisciplinary teams.
  • Maintain confidentiality and adhere to policies and procedures.
  • Report housing issues and assist in resolving defects.
  • Undertake on-call duties and participate in reception duties as required.
