Job summary

Carers’ Resource in Harrogate is seeking a Young Carer Practitioner (16–25) to support the wellbeing, education and employability of young carers across Harrogate, Craven and Selby districts. The role involves one-to-one support, mentoring and group activities, with a whole-family approach to identify needs and barriers.

Responsibilities

• Assess the needs of young carers within the context of their family circumstances.
• Work with the whole family to identify barriers, challenges and unmet needs.
• Provide one-to-one information, advice, mentoring and emotional support.
• Provide targeted support to young carers aged 16–25 who are NEET, helping them identify and overcome barriers to education, employment and training.
• Support young people to develop confidence, independence, aspirations, employability skills and realistic progression plans.
• Help 16–25-year-old young carers access appropriate education, training, apprenticeships, employment, volunteering and other positive opportunities.
• Work collaboratively with education, employment, training, careers and specialist support services to improve outcomes for young carers aged 16–25.
• Develop family‑focused support plans that reduce inappropriate caring responsibilities and enable young people to pursue education, employment or training.
• Plan and deliver age‑appropriate group activities, including activities that promote wellbeing, skills, confidence, aspirations and social connection.
• Monitor and review young carers’ needs, including changes within the family and transitions between education, training and employment.
• Work collaboratively with statutory, voluntary and community organisations.
• Make appropriate referrals and signpost families and young people to additional services, including specialist education, employment, training, careers and transition support.
• Maintain accurate and confidential case records.
• Follow safeguarding procedures and respond appropriately to concerns.
• Seek feedback from young carers and families to inform service improvement.
• Act as an ambassador for Carers’ Resource at all times.
