This is anexciting opportunity to join Carers’ Resource, a growing and thriving charitywhich supports unpaid carers: people who are looking after a friend, neighbouror relative who could not manage otherwise because of frailty, illness ordisability. Carers’ Resource support young carers and adult carers as well as providingseveral hospital services, our Home from Hospital teams work across NorthYorkshire and Bradford District, we help people to be more confident and comfortablewhen returning home from hospital. This service can ease a return home, helpingrebuild confidence and independence, and preventing hospital re-admission. TheCarer Navigator service supports friends and families who have a loved oneadmitted to Airedale General Hospital or Bradford Royal Infirmary. We have ateam of staff, who are dedicated to supporting people who are looking after arelative or friend in hospital.

We are looking for a Young Carer Practitioner (16-25) working across the Harrogate, Craven and Selby districts.

Role overview:

As a Young Carer Practitioner, you will support the social, emotional, educational and vocational wellbeing of young adult carers across Harrogate, Craven and Selby, of young people aged 16–25. You will provide a combination of one-to-one support, mentoring, information and advice, and group/activity-based work.

Central to this role is embedding a whole family approach. You will recognise the young carer as part of a wider family system, work proactively with parents, siblings and wider family members, identify additional support needs, and collaborate with colleagues and partner agencies to ensure coordinated, family-centred support.

For young carers aged 16–25, you will provide targeted support to help them overcome barriers to education, employment and training, develop confidence and independence, access appropriate opportunities, and make positive transitions into adulthood. This may include identifying NEET-related barriers, supporting employability and skills development, helping young people access education, training apprenticeships, volunteering or employment, and working with relevant specialist services and partner organisations.

The post requires flexibility, including some early evening work and participation in activities during school holidays.

Requirements or Key Responsibilities:

• Assess the needs of young carers within the context of their family circumstances.
• Work with the whole family to identify barriers, challenges and unmet needs.
• Provide one-to-one information, advice, mentoring and emotional support.
• Provide targeted support to young carers aged 16–25 who are NEET, helping them identify and overcome barriers to education, employment and training.
• Support young people to develop confidence, independence, aspirations, employability skills and realistic progression plans.
• Help 16–25-year-old young carers access appropriate education, training, apprenticeships, employment, volunteering and other positive opportunities.
• Work collaboratively with education, employment, training, careers and specialist support services to improve outcomes for young carers aged 16–25.
• Develop family-focused support plans that reduce inappropriate caring responsibilities and enable young people to pursue education, employment or training.
• Plan and deliver age-appropriate group activities, including activities that promote wellbeing, skills, confidence, aspirations and social connection.
• Monitor and review young carers’ needs, including changes within the family and transitions between education, training and employment.
• Work collaboratively with statutory, voluntary and community organisations.
• Make appropriate referrals and signpost families and young people to additional services, including specialist education, employment, training, careers and transition support.
• Maintain accurate and confidential case records.
• Follow safeguarding procedures and respond appropriately to concerns.
• Seek feedback from young carers and families to inform service improvement.
• Act as an ambassador for Carers’ Resource at all times.
