An opportunity has arisen for an enthusiastic and highly motivated DSN to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. This mobile service includes a Dietician, Clinical Psychologist, Youth Worker, Social Prescriber, and administrator.

This new post aims to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. The post holder will support young adults facing difficulties related to living with diabetes, especially during their transition to adult services. The work involves supporting individuals with both Type 1 and Type 2 diabetes.

The post offers scope to develop the service with support from senior colleagues and collaboration with service users.

The role is based at Mile End Hospital but involves working across all Barts Health sites, including Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital, ensuring high-quality, standardized care across the Trust.

We welcome applications from qualified diabetes specialist nurses with a strong interest in working with this patient group.

As a member of the core young adult MDT, the successful candidate will join the wider Barts Health diabetes service, supported by senior clinicians. This is an exciting opportunity to be part of an innovative service focused on improving diabetes care and outcomes in young people with both Type 1 and Type 2 diabetes across Barts Health.

The primary role involves supporting high-risk young adults, working with social workers, youth workers, and psychologists to safeguard these patients and engage them effectively.

The post holder will develop specialist skills to provide management plans, treatment advice, and support for young adults with diabetes and their carers.

The role involves working across all sites to deliver specialist care and support to young people, their families, and clinical teams. The post also includes being a clinical and educational resource for patients and health professionals, always striving to improve diabetes care.
