Job summary

Pilgrims Hospices is seeking a Wellbeing Practitioner to join a multidisciplinary team providing psychosocial care for patients and their families. You will assess wellbeing, develop tailored support plans, and link individuals to services that improve independence and quality of life.

The role involves compassionate conversations, group and one-to-one interventions, and contributing to service development.

Qualifications

• GCSEs in Maths and English are required.
• NVQ Level 2 and 3 in Health and Social Care are desirable.
• Further relevant qualifications in therapeutic support are desirable.

Responsibilities

• Undertake person-centred holistic assessment with patients, carers and families.
• Engage with Wellbeing Programmes, providing group, 1-1 and virtual interventions.
• Support the ongoing development and evaluation of wellbeing services.
• Promote wellbeing programmes to patients and families and signpost to external groups.
• Liaise with local community services and multidisciplinary teams to support care.

Education

GCSE Math and English (minimum C)
