Job summary

Pilgrims Hospices in East Kent is seeking a Wellbeing Practitioner to join the Thanet team on a part-time basis. The role focuses on delivering psychosocial care through group and individual wellbeing interventions to improve quality of life, resilience, and psychological wellbeing for patients and carers.

You will work 30 hours per week at the Thanet Hospice site, with shifts 8am-4pm or 8.30am-4.30pm, and you will contribute to a broad range of wellbeing programmes while supporting

Qualifications

• Experience delivering psychosocial care to patients and carers.
• Ability to work in a multi-disciplinary setting.
• Flexibility to work part-time (30 hours/week) and to follow set patterns.

Responsibilities

• Undertake person centred holistic assessment with patients, carers and families.
• Engage with Wellbeing Programmes; facilitate group and 1-2-1 interventions.
• Promote Wellbeing Programmes to patients and families; encourage referrals.
• Support the service with administrative duties as required.
• Participate in Multidisciplinary Team meetings and share information for patient outcomes.
