Job summary

Catch22 is hiring a Wellbeing Practitioner to deliver holistic wellbeing support in Bristol. The role focuses on assessments, plan development, and coordinated, multi-agency interventions to improve clients’ emotional resilience and daily living skills.

You will travel within the Bristol area, collaborate with partners, and maintain GDPR-compliant records. A valid UK driving license is essential, and a full-time contract is offered with a salary band around £26k.

Qualifications

• Valid driving license and access to a reliable vehicle.

Responsibilities

• Deliver personalized wellbeing interventions and support programs tailored to individual service user needs across multiple wellbeing pathways.
• Develop and implement evidence-based strategies to help service users manage emotions, build self-confidence, and foster emotional resilience.
• Provide skills training and coaching to empower service users in managing daily living activities and building independence.
• Conduct thorough assessments of service user needs and create comprehensive wellbeing plans in collaboration with clients.
• Advocate for service users and coordinate with multi-agency partners, including health professionals, support groups, and community organizations.
• Build and maintain strong, trusting relationships with service users through empathetic and transparent communication.
• Maintain detailed and accurate records of service user progress, interventions, and outcomes while ensuring confidentiality and GDPR compliance.
• Collaborate with internal team members and external stakeholders to achieve organizational objectives and deliver measurable outcomes.
• Identify and connect service users with appropriate resources, support services, and community programs.
• Participate in ongoing professional development, training, and team meetings to enhance service delivery.
• Travel within the Bristol area and to partner locations as required to deliver services and attend meetings.

**Preferred**

• Relevant qualification or certification in rehabilitation, criminology, criminal justice, social care, mental health, information and guidance, family services, emotional wellbeing, or social inclusion
• Experience working within the criminal justice system or with individuals with complex needs
• Knowledge of wellbeing frameworks and evidence-based intervention models
• Experience with service user outcome tracking and performance reporting
• Familiarity with multi-agency working and partnership approaches

Successful post-holders will need to have a full UK Driving License and access to a personal vehicle for work purposes.
Contract: Permanent
Hours & work pattern: Full time 38 hours per week.
Salary band: £26,097.00
