Derbyshire Community Health Services NHS Foundation Trust is seeking a Weight Management Adviser to join the Specialist Tier 3 Weight Management Service in Chesterfield. You will contribute to evidence-based weight management and work within a multidisciplinary team to support adults with obesity.

The role involves delivering assessments, coaching, and group sessions, with opportunities to develop patient resources and digital care approaches in a supportive environment.

Qualifications

• Level 3 exercise referral qualification (or equivalent).
• Level 3 nutrition and weight management qualification (or equivalent).
• Training in motivational interviewing/behaviour change/counselling or relevant experience.
• Level 4 obesity management qualification or equivalent experience.

Responsabilités

• Deliver evidence-based weight management assessments, education, advice and ongoing support to adults with complex obesity.
• Manage a caseload of patients receiving specialist Tier 3 weight management support, providing personalised coaching and behaviour change interventions.
• Work collaboratively with the wider multidisciplinary team, ensuring patients access specialist dietetic, psychological and medical support when appropriate.
• Facilitate and support both face-to-face and virtual group education sessions.
• Support patients to set realistic goals, monitor progress and identify barriers to sustainable lifestyle change.
• Collect, record and interpret outcome measures in line with service requirements and quality standards.
