Job summary

Fairview Hospital / Cleveland Clinic is seeking a Certified Nurse Assistant to provide home care services on Saturdays, 9:30 AM to 12:30 PM. The role focuses on direct patient care, hygiene, transfers, meal prep, companionship, and documentation in EVV systems.

Responsibilities include monitoring patient conditions, following the care plan, and ensuring privacy and safety. Travel within the designated area and independent work within the plan are required.

Qualifications

• Certified Nursing Assistant (CNA) certification current with DFS.
• High school diploma or GED preferred.
• Strong communication skills; able to read, write, and follow instructions.
• Physically able to perform lifting, standing, and transfers.
• Sympathetic, patient-centered approach to care.

Responsibilities

• Deliver direct patient care including personal hygiene, transfers, repositioning, and ambulation assistance.
• Provide basic nursing care and support prescribed therapies under supervision.
• Prepare meals according to dietary needs and preferences.
• Offer companionship and engage in activities tailored to individual interests.
• Perform light housekeeping and maintain accurate records.
• Monitor and report changes in the patient’s condition.
• Document tasks using EVV and adhere to the care plan, without administering medications.
• Ensure patient privacy, confidentiality, and safety; report concerns or abuse.
• Support patient independence and collaborate with team members.
