Qualifications

• Good standard of English, both written and spoken.
• Must have a valid driver’s licence and access to own car.

Responsibilities

• Provide personal care and daily living support in line with each service user’s care plan.
• Respect service users’ dignity, independence and choices.
• Maintain professional relationships with service users, families and healthcare professionals.
• Report changes in a service user’s condition to the supervisor.
• Follow Connected Health policies, safety requirements and on-call guidance.
• Complete all required documentation including care records and timesheets.
• Attend induction, training, supervision and meetings.
• Undertake additional duties as requested by the Service Manager.
