Job description

Are you a nurse looking for a new challenge? Do you want to enable patients to enjoy maximum independence and optimum quality of life?


The successful postholder will be accountable to the Senior Virtual Ward nurse and will have continuing responsibility for the nursing care given to patients with sub-acute conditions in identified community settings, without direct supervision. The nurse will be personally accountable for the nursing standards of such delegated duties. The nurse will also be responsible for demonstrating procedures for qualified and/or unqualified staff; and will provide evidence-based care, advice and information to patients and carers.


You will be employed by North Middlesex University Hospital (Enfield Community) with primary responsibility at NMUH Virtual Ward. Occasionally the post holder may be expected to work across the wider VW team. You will need to hold a full driving licence and have access to a car for work.


The successful post holder will proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and facilitate early discharge to improve patient health outcomes. Patients requiring case management in the Rapid Response and Virtual Ward service will be identified by case-finding in the Same Day Emergency Care (SDEC) unit, Accident and Emergency department, NMUH wards and also via Community referrals e.g., GP, Care Homes, Community Nurses, and LAS.

We also provide community services and have a dedicated 0-19 service for children and young people in Enfield so that they can get the best possible start in life. This includes health visitors and school nurses who are delivering the national Healthy Child Programme, which provides a structured framework for the delivery of key interventions to support the health and wellbeing of children and families from 0 to 5 and school aged children from 5-19. Clinical/ Professional Role:
  1. Provide specialist, high quality, expert nursing care and support for patients who are suitable for Virtual Ward (VW) care in liaison with the multidisciplinary teams (MDT) through assessment and care planning.
  2. Initiate agreed range of diagnostic tests.
  3. Refer to other disciplines/professions when required.
  4. Support patients to self-manage, monitor their condition, and make informed choices.
  5. Act as a named nurse for designated patients within the service.
  6. Ensure that nursing documentation is maintained to required standards and to be a competent user of and participate in the ongoing development of the Trust’s planned Electronic Patient Record.
  7. Promote the development of evidenced based practice.
  8. Participate in clinical supervision to agreed standard.
  9. Ensure high standards of care are given to all patients.
  10. Be responsible for gaining intravenous access and administration of prescribed medication for patients requiring short-term antibiotics and for monitoring their care.
  11. Collaborate with medical colleagues in ensuring that appropriate investigations and procedures are performed to complete the patients’ treatments and that patient, and their families are fully informed and understand the purpose for these.
  12. Advise patients on the promotion of health and the prevention of illness.
