As a Virtual Care Practitioner, you will provide enhanced clinical assessment and management of complex, high-risk and undifferentiated patient presentations, helping patients access the right care, in the right place, at the right time. Working within our Integrated Care Contact Centres, you will utilise advanced clinical reasoning, professional judgement and evidence-based practice to support safe, effective and patient-centred care.
Alongside delivering expert clinical care, you will play a key role in the development of colleagues, providing clinical leadership, mentorship and support across the service. You will work collaboratively with multidisciplinary teams and healthcare partners, helping to shape clinical pathways, improve patient outcomes and influence how Virtual Care develops across the Trust.
