As a Virtual Care Practitioner, you will manage complex, high-risk and undifferentiated patient presentations, providing expert clinical decision‑making and senior support within our Virtual Care service. Alongside delivering exceptional patient care, you will provide leadership, mentorship and clinical guidance to colleagues, whilst contributing to service development, innovation and quality improvement.
As a Virtual Care Practitioner, you will:
Undertake enhanced virtual assessment and management of patients accessing 999, NHS 111 and other referral pathways.
Manage complex and high-risk patients using advanced clinical reasoning, evidence‑based practice and professional judgement.
Formulate differential diagnoses and develop safe, effective and patient‑centred management plans.
Provide expert clinical advice, leadership and support to colleagues across the Virtual Care service.
Lead, coach and mentor clinicians, supporting professional development and workforce capability.
Support the development and optimisation of referral pathways and innovative models of care.
Contribute to quality improvement, clinical governance, education and service transformation initiatives.
Work collaboratively with multidisciplinary teams and healthcare partners to improve patient outcomes across the healthcare system.

Please note that the salary range noted on the vacancy is in line with agenda for change (AFC) pay scales. All successful applicants would be placed automatically at the bottom of the banding, unless proven, relevant NHS or equivalent experience can be demonstrated.
