Job overview

The Oxford University Hospital wishes to appoint a part time ( 22.5 hours per week ) Band 6 Vascular Nurse Practitioner to work within the vascular triage unit.

The triage unit receives urgent referrals from GPS and Consultant clinics and other hospitals within the vascular network. The role will encompass effective assessment and examination skills, and will prompt requests for future investigation or treatments for patients .

This ensures that patients with a broad range of vascular conditions such as diabetic foot ulcers, critical limb ischaemia or post stroke, receive prompt and individualised care from their arrival within on the ward.

This role supports the Vascular Registrar and Consultant with triaging vascular patients from the through their care pathway. We offer a structured orientation programme and you will work along side an experienced Vascular Nurse Practitioner in order to develop your knowledge and skills in taking patients’ clinical history and physical assessment. There will be opportunities available for the successful candidate to undertake relevant courses .

The following knowledge is desirable for the postholder: ·

  • Autonomous practice experience supported by the appropriate post registration qualification.
  • Evidence of experience using advanced clinical examination and assessment skills within practice supported by appropriate qualifications
  • Have a broad knowledge of complex and common Vascular conditions.
Main duties of the job

The overall purpose of the Vascular Nurse Practitioner role is

  • Responsible for initial assessment, examination, initiation of treatments, including medication and investigations for vascular service patients.
  • Provide advice, education and support to patients, their carer’s and other health care professionals.
  • Assess individuals, families and populations holistically using a range of different methods, some of which may not be usually exercised by nurses such as physical assessment and history taking, ordering and interpreting diagnostic tests or advanced health needs assessments.
  • Practice clinically in agreed care settings according to approved protocols or guidance.
  • Contribute to the smooth running of the designated service.
  • Contribute to the on-going development of the designated service.

Detailed job description and main responsibilities

The Vascular Nurse Practitioner will;

  • Work collaboratively and in partnership with other health care professionals, offering advice to colleagues.
  • Assess health and psychosocial needs of individual patients, and provide care to meet these needs, through a process of shared decision making.
  • Assess and evaluate, with patients and /or carers, the effectiveness of the treatment and care provided and make changes as needed
  • Deliver expert vascular nursing care to patients in agreed practice settings, working as part of nursing/multidisciplinary teams.
  • Communicate with patients in ways that empower them to make informed choices about their health and health care.
