Job summary

Norfolk Community Health and Care NHS Trust in Norwich Place is recruiting an experienced Occupational Therapist or Physiotherapist to join the UCR Therapy Team. You will contribute to rapid response triage, admission avoidance and discharge to assess for our community-based patients.

You will assess, plan and deliver evidence-based therapies, liaise with GPs and social care, and participate in a 7-day service with 8am-6pm shifts and weekend rota.

Qualifications

• Experience as an Occupational Therapist or Physiotherapist.
• Experience in urgent/community settings is preferred.

Responsibilities

• Undertake comprehensive therapy assessments using clinical reasoning.
• Formulate and execute individualised therapy plans with the team.
• Collaborate with GPs, social services and inpatient teams to coordinate care.
• Ensure documentation on Systm One is updated daily.
