Job summary

Norfolk Community Health and Care NHS Trust is seeking an experienced Occupational Therapist to join the Norwich Community Therapy team, based at Norwich Community Hospital. You will work with a flexible, independent approach to deliver a high-quality service and manage a varied caseload including neurology, falls, orthopaedics, and long-term conditions, with triage support.

The post requires self-motivation, strong clinical reasoning, and excellent communication skills.

Responsibilities

• Undertake comprehensive occupational therapy assessments of patients.
• Formulate and execute individualized treatment plans.
• Assess functional ability and safety of patients in the community.
• Assess & plan treatment including teaching patients to utilise techniques & equipment.
• Liaise with GPs, Social Services and other health professionals to coordinate care.
• Plan and organise specialist services within the team.
• Ensure patient records are completed and input into SystmOne daily.
