Job Description

Service Care Solutions are recruiting Urgent Community Response Practitioners to join a busy NHS team on a full-time agency basis. This role offers an excellent opportunity for clinicians experienced in urgent care and crisis intervention in the community.

Responsibilities
  1. Respond rapidly to patients in crisis within the community to prevent unnecessary hospital admissions.
  2. Work independently and with a multidisciplinary team to assess, treat, and support patients at home or in care settings.
  3. Provide patient-centered care focusing on recovery, safety, and independence.
  4. Coordinate with GPs, social care, community teams, and hospitals to ensure seamless care.
Candidate Requirements
  • Qualified Registered Nurse, Paramedic, or Allied Health Professional.
  • Experience in urgent care, community health, crisis response, or A&E.
  • Ability to work independently and manage caseloads under pressure.
