? Band 7 Urgent Community Response Practitioner – Full-Time NHS Agency Contract ?
Location: West London
Pay Rate: £32.00 per hour (via umbrella)

Service Care Solutions are currently recruiting for a Band 7 Urgent Community Response Practitioner to join an NHS team on a full-time agency basis. This is an exciting opportunity for a skilled and autonomous clinician to contribute to an urgent care pathway that helps reduce hospital admissions and supports patients safely in the community.

? Role Overview: As a Band 7 Practitioner, you will:

  • Deliver rapid-response clinical assessments and interventions to patients in their own homes or care settings.

  • Work as part of a multidisciplinary urgent care team to stabilise, support, and manage acute health needs.

  • Promote hospital admission avoidance and support early discharge from acute care.

  • Take a lead role in clinical decision-making, care coordination, and supervision of junior staff.

? Requirements:
  • HCPC/NMC registered clinician (Nurse, Paramedic, AHP).

  • Strong clinical background in urgent/emergency care, community health, or rapid response.

  • Proven leadership and case management skills at Band 6/7 level.

  • Ability to work independently and make safe, effective decisions under pressure.
