Job summary

Torbay and South Devon NHS Foundation Trust invites applications for a community-based support worker role, covering Torbay and South Devon. The role focuses on helping people stay in their own homes, delivering short-term care after hospitalisation and enabling independence.

You will work across a 7-day rota including days, evenings and bank holidays, with a supportive team dedicated to high-quality domiciliary care and collaboration with professionals to tailor interventions to patient needs.

Responsibilities

  • Provide personal care and monitor the patient's condition.
  • Enable and assist individuals to remain independent at home.
  • Carry out non-nurse tasks as appropriate within the service user’s plan.
  • Collaborate with other support workers and professionals to provide care.
  • Provide high-quality domiciliary support to promote independence.
  • Assess risk and needs following the care plan.
  • Liaise with professionals to ensure appropriate support and suggest improvements.

Job description

We have an opportunity to come and work for the Rapid Response Team, covering the whole of Torbay and South Devon, assisting people to remain in their own homes. We are looking for a motivated member of staff to work as part of a friendly, hardworking & committed team, working across 7 days to provide short term care and support to people in the community. We focus on supporting and enabling patients to live independently at home and maximise independence, following a period of acute ill health or hospitalisation. If you can demonstrate your flexibility, community experience and like to be able to work autonomously, providing interventions for patients within their own homes then this could be the opening for you. Duties involved will include:

  • provision of personal care, clinical duties where delegated, monitoring the patient's condition
  • To enable and / or assist individuals to remain in the community, to be supported and maintained safely in their own homes and thereby enhance the quality of life.
  • To carry out the personal tasks which are not the responsibility of a nurse but would normally expect to be undertaken by a member of the service user's family.
  • Collaboration with other support workers and professionals to provide care for the service user
  • To provide good quality domiciliary support to the service user in their own home, promoting independence.
  • Assess the risk and needs of service user's in accordance with the care plan.
  • The role is very varied as such you will need to liaise with other professionals to ensure the patients are supported in the most appropriate way and provide suggestions to improve support ongoing

The services are provided by Support Workers who visit during the day from 8:00 am till 22:00 pm operating on a 7 day rota system, including evenings, weekends and Bank Holidays.
