Job summary

Sandwell Metropolitan Borough Council is seeking a Trusted Triage Assessor to assess adults in their homes and arrange appropriate low-level equipment and daily living aids. You will deliver, fit and maintain equipment, provide demonstrations on safe use and keep accurate records to support wellbeing.

The role involves risk assessments, referrals, and collaborating with health and social care partners to improve independence and outcomes for residents.

Qualifications

  • Experience working with vulnerable adults.
  • Experience within a repair, maintenance, installation or equipment-related environment.
  • Literacy Level 2 qualification (or equivalent).
  • Full UK driving licence and ability to drive a long wheel based vehicle.

Responsibilities

Job Title: Truster Triage Assessor

Grade/Salary: Band D, SCP 9-17: £27,254- £31,022 per annum

Contract Type: Permanent

Hours: 37 hours per week (service operates 7 days a week, including Bank Holidays). Successful candidates will work on a rota basis, including one weekend every six weeks.

Location: Joint Prevention Stores, Dolton Way, Tipton DY4 9AL

Supporting Independence Through Assessment, Advice and Equipment Provision

Do you have experience working with vulnerable adults and a passion for helping people maintain their independence?

We are looking for a compassionate and motivated Trusted Triage Assessor to join our Joint Equipment Stores team. This rewarding role combines assessment, customer-focused support and the delivery of low-level daily living aids to help people remain safe, independent and confident in their own homes.

About The Role

As a Trusted Triage Assessor, you will undertake assessments with adults to identify their needs and provide or recommend appropriate low-level equipment and daily living aids.

You will work directly with residents in their homes, providing practical solutions that improve wellbeing and prevent or delay the need for more intensive services.

We are seeking an individual who is committed to excellent customer service and improving the lives of vulnerable adults.

Essential Requirements
  • Experience working with vulnerable adults.
  • Experience within a repair, maintenance, installation or equipment-related environment.
  • Literacy Level 2 qualification (or equivalent).
  • Full UK driving licence and ability to drive a long wheel based vehicle.
  • Strong communication and interpersonal skills.
  • Good IT and record-keeping skills.
  • Knowledge of safeguarding, customer care and safe working practices.
  • Ability to manage a varied workload and work independently as well as part of a team.
