Job summary

Responsibilities

• Build trusting, supportive relationships with young people facing complex challenges.
• Deliver one-to-one mentoring and practical support in community settings.
• Help young people understand needs, strengths and set achievable goals.
• Support transitions to independent living, education and housing.
• Collaborate with families, carers, social workers and other professionals.
• Monitor progress and maintain accurate records of outcomes.
