Job summary

Belmont Recruitment is recruiting for a Hybrid Transitional Social Worker role with Peterborough Council. The position is based at Sand Martin House, Peterborough, with 2 days in office and 3 days from home.

You will undertake Care Act 2014 assessments, reviews and Best Interest Decisions, supporting young people 16–25 as they transition to adulthood, and work as part of a Multidisciplinary Team.

Qualifications

• Experience with Care Act 2014 assessments and best interests decisions.
• Knowledge of Transition from children to adults and SEND.
• Ability to travel independently between locations.

Responsibilities

• Complete Care Act 2014 assessments and reviews.
• Support transitions for 18–25 year olds and coordinate with MDTs.
• Travel between locations including out-of-county as needed.

Hybrid - Transitional Social Worker - Peterborough Council

Salary: £246.16 Per Day

Hours: 37 Hours

Working Schedule: Monday to Friday

2 days in Office, Monday mandatory and 3 days from home (Sand Martin house)

The role involves completing Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) identifying individuals needs and supporting the Multidisciplinary teams' recommendation on eligibility for full NHS Continuing Healthcare.

You will have experience and/or knowledge of supporting people who are undergoing Transitions from children to adults, Experience and/or knowledge of The Care Act 2014, 0 -25 Special Education

Needs and Disability (SEND Agenda) Community Support, the National Framework for Children and Young People's Continuing Care (2016), and Children's Act 1989 where young people are transitioning into adulthood.

The successful candidate will assist families to access activities and services, Advice, advocating and will need to be able to travel independently between locations including out of county areas on occasions.

We are a Transition Team and we support young people from 18 - 25 years completing the Care Assessments, however we start getting to know young people from the age of 16 years.

We also complete COP DOLS and MCA's. A person with both Legislative experience of children's and adults will be beneficial to the Team.
