Job summary

Peterborough City Council is seeking an experienced Social Worker to join the 0-25 Transition Team in Peterborough. You will support young people moving from Children's Services into adulthood, conducting Care Act assessments and guiding families through the transition process.

The role requires experience in Transition or Adult Social Care, knowledge of the Care Act 2014, and the ability to travel as needed. Hybrid working with 2 office days and 3 home days is offered.

Qualifications

• Social Work qualification and professional registration.
• Experience in a Transition Team or Adult Social Care.
• Strong knowledge of the Care Act 2014.
• Understanding of SEND and Children Act 1989.
• Ability to travel independently for work.
• Excellent assessment and partnership-working skills.

Responsibilities

• Complete Care Act 2014 assessments and reviews.
• Undertake MCA assessments and Best Interest decisions.
• Complete Court of Protection DoLS and MCA-related work.
• Support multidisciplinary teams in determining eligibility for NHS Continuing Healthcare.
• Assist young people and families in accessing services, support, and community activities.
• Provide advice, advocacy, and guidance throughout the transition journey.
• Travel independently across locations, including occasional out-of-county visits.
