Job summary

Peterborough City Council is seeking an experienced Social Worker to join their 0-25 Transition Team. This role supports young people moving from Children's Services into adulthood, coordinating assessments and services to achieve positive outcomes.

Based in Peterborough with flexible hybrid working (2 office days, 3 home days), you will assess needs under Care Act 2014, work with MCA and DoLS, and help families navigate the transition. You may travel to different locations as required.

Qualifications

• Qualified social worker with professional registration.
• Experience in Transition or Adult Social Care teams.
• Strong knowledge of Care Act 2014.
• Understanding of SEND and Children Act 1989.
• Experience supporting 16-25 transition and collaboration with families.
• Willingness to travel as needed.

Responsibilities

• Complete Care Act 2014 assessments and reviews.
• Undertake MCA assessments and Best Interest decisions.
• Do COP DoLS and MCA-related work.
• Assist eligibility for NHS Continuing Healthcare.
• Support young people and families to access services.
• Provide advice, advocacy, and guidance through transition.
• Travel across locations, including occasional out-of-county.
