Job summary

Turning Point is launching a brand-new residential rehabilitation and Discharge to Assess service in England and seeks a Lead Recovery Worker – Transition Lead to shape The Stewarts and ensure safe, planned transitions from admission to move-on.

You will lead admissions, transition and discharge planning, working with housing providers, health professionals and local authorities to maximise independence and outcomes, while mentoring Recovery Workers across the service.

Qualifications

• Experience in mental health, social care, supported housing or community services.
• Experience supporting transitions, move-on or discharge planning.
• Knowledge of housing pathways and community resources.
• Experience with complex mental health and social needs.
• Strong communication, leadership and relationship-building skills.
• Safeguarding, risk management and professional boundaries.
• Ability to work with internal and external stakeholders.
• Excellent organisational and problem-solving skills.

Responsibilities

• Lead on admissions, transition planning, discharge planning and move-on pathways across the service.
• Ensure move-on planning begins at admission and remains a clear focus throughout a person’s stay.
• Develop personalised transition plans.
• Coordinate pathways into supported accommodation, independent living and community services.
• Maintain positive relationships with housing providers, local authorities and community partners.
• Identify and overcome barriers to timely moves and positive outcomes.
• Monitor bed flow, vacancies and transition outcomes.
• Provide guidance and support around move-on planning and community integration.
• Lead projects to improve independence, recovery outcomes and social inclusion.
• Maintain records and support audits and continuous improvement.
