Job summary

Turning Point UK is launching The Stewarts, a residential service with Intensive Enablement and Discharge to Assess pathways. The Transition Lead will head admissions, move-on planning and discharge pathways to embed recovery goals from day one.

You will collaborate with housing providers, health professionals, local authorities and community services to ensure timely access to right support. A percentage of time is dedicated to Transition Lead duties.

Qualifications

• Experience within mental health, social care, supported housing or community services.
• Experience supporting individuals through transitions, move-on or discharge planning processes.
• Strong communication, leadership and relationship-building skills.
• Knowledge of safeguarding, risk management and professional boundaries.

Responsibilities

• Lead admissions, transition planning, discharge planning and move-on pathways across the service.
• Ensure move-on planning begins at admission and remains a clear focus.
• Develop personalised transition plans with individuals based on aspirations and strengths.
• Coordinate pathways into supported accommodation, independent living and community-based services.
• Maintain relationships with housing providers, local authorities and community partners.
• Identify and overcome barriers delaying moves and impacting outcomes.
• Monitor bed flow, vacancies and transition outcomes to support service delivery.
• Provide guidance and support around move-on planning and community integration.
