Job description

Join us as we launch a brand-new residential rehabilitation and Discharge to Assess service supporting people with complex mental health needs to achieve greater independence and successful move-on outcomes. At Turning Point, we believe recovery is possible for everyone. We are seeking an experienced and motivated Lead Recovery Worker - Transition Lead to help shape and develop The Stewarts, an innovative residential service providing both Intensive Enablement and Discharge to Assess (D2A) pathways. As the Transition Lead, you will take the lead in ensuring people experience safe, planned and successful transitions throughout their recovery journey. From admission through to move-on, you will coordinate pathways that promote independence, reduce delays and support people to achieve sustainable outcomes in the community. Passionate about helping people move forward in their recovery, you will work closely with housing providers, health professionals, local authorities and community services to ensure people have access to the right support at the right time. Please note, this is a Recovery Worker role with a percentage of time spent on Transition Lead duties.

Main Responsibilities As a Lead Recovery Worker - Transition Lead, you will:
  • Lead on admissions, transition planning, discharge planning and move-on pathways across the service.
  • Ensure move-on planning begins at admission and remains a clear focus throughout a person's stay.
  • Support people we support to identify their aspirations, strengths and future goals, developing personalised transition plans.
  • Coordinate effective pathways into supported accommodation, independent living and community-based services.
  • Develop and maintain positive relationships with housing providers, local authorities, commissioners and community partners.
  • Identify and overcome barriers that may delay successful moves or impact positive outcomes.
  • Monitor bed flow, vacancies and transition outcomes to support effective service delivery.
  • Support people through periods of change, helping them build confidence and resilience for the next stage of their recovery journey.
  • Work collaboratively with multidisciplinary professionals to ensure continuity of care and coordinated support.
  • Monitor transition outcomes and use data, feedback and learning to improve pathways and service performance.
  • Act as a specialist resource for colleagues, providing guidance and support around move-on planning and community integration.
  • Lead projects and initiatives that improve independence, recovery outcomes and social inclusion.
  • Maintain accurate records and contribute to audits, quality assurance activities and continuous improvement initiatives.
  • Provide day-to-day leadership, mentoring and support to Recovery Workers where appropriate.
    We are looking for an experienced and motivated practitioner who is passionate about helping people progress through their recovery journey and achieve greater independence., Experience working within mental health, social care, supported housing or community services.
  • A strong understanding of recovery-focused and person-centred approaches.
  • Experience supporting individuals through transitions, move-on or discharge planning processes.
  • Knowledge of housing pathways, community resources and support options.
  • Experience working with people who have complex mental health and social needs.
  • Strong communication, leadership and relationship-building skills.
  • Knowledge of safeguarding, risk management and professional boundaries.
  • Experience working collaboratively with internal and external stakeholders.
  • Excellent organisational and problem-solving skills.
  • Confidence using digital systems and maintaining accurate records.
  • A commitment to inclusion, equality and continuous improvement.
  • A Level 3 qualification in Health & Social Care (or equivalent) is desirable but not essential.
