Job summary

Surrey and Borders Partnership NHS Foundation Trust in Chertsey is seeking a Community Perinatal Mental Health Practitioner to support women during pregnancy and postnatally, alongside their babies and partners.

You will assess, plan, implement and evaluate care for a defined caseload, mentor staff, and contribute to a multidisciplinary team, with opportunities for ongoing training and professional development.

Qualifications

• Degree in health/social care or equivalent qualification/experience.
• Registered Mental Health Nurse, Occupational Therapist, or Social Worker.
• Evidence of continued professional development.

Responsibilities

• Be accountable for a defined caseload and assess care needs under supervision.
• Manage referrals, triage and assessments within specified timescales.
• Respond to emergency referrals and liaise with HTT, inpatient services and MBU as needed.
• Contribute to multidisciplinary assessment with a perinatal mental health nursing perspective.
