Trafford Council is seeking a dedicated Family Hub practitioner to work with children, young people and families. You will be part of the Family Hub Team, coordinating support and delivering evidence-based programmes in collaboration with education colleagues and partner agencies.
The role focuses on identifying needs, planning interventions, and ensuring a whole-family approach is implemented effectively across services. DBS checks are required, with on-site working at hubs or schools.

This is an exciting opportunity for you to join our Family Hub Team. Trafford Team Together is our team around the school approach so you would be working very closely with education colleagues and wider partners. We are committed to delivering the right support at the right time, and in the most effective way for children and families, through a whole family approach to assessment, planning and delivery of evidence based programmes.
You will bring experience of working with children and families through an early help approach. You will have skills in identifying and assessing the needs of children, young people & families, planning, coordinating and leading support and intervention as a key worker. You will also have experience of working closely in partnership with a range of partners to ensure there is a whole family team around approach’. You will have experience of working with diverse communities and a commitment to ensuring that children, young people and families achieve the best possible life outcomes.
An enhanced DBS check with children/adults barred list check will be required.
This post is not a hybrid working role – you will be based in the Hub or schools.
