Kent Community Health NHS Foundation Trust is seeking a skilled Wound Clinic Nurse to assess, treat and plan care for patients with wounds in our tissue viability service. You will run clinic sessions, supervise Healthcare Assistants and deputise for the Senior Wound Therapist.

Requirements include Level 6 degree or equivalent with a nursing diploma, NMC registration, and recent healthcare experience. Excellent communication, IT literacy and the ability to travel between clinics are also

Qualifications

• Ability to use IT systems and databases.
• Knowledge of where to signpost clients and families to alternative services.
• Up-to-date clinical practice and adherence to evidence-based procedures and clinical effectiveness frameworks.
• Understanding of the role and remit of a healthcare worker.

Responsabilités

• Assess, treat and monitor patients attending wound clinics using evidence-based wound care
• Carry out comprehensive wound assessments and develop individualised care plans with patients and the multidisciplinary team
• Evaluate treatment outcomes and prescribe appropriate wound therapies
• Provide education, advice and support to patients, families and carers on healing and self-management
• Communicate with GPs, community services, care homes, hospitals and other healthcare professionals to coordinate care
• Organise and prepare clinic sessions, prioritising workload and managing appointments
• Support the day-to-day running of wound clinics
• Deputise for the Senior Wound Therapist when required
• Provide leadership and supervision to Healthcare Assistants
• Support students and visitors
• Champion safe, compassionate, high-quality care in line with current best practice
