The East of England Community Health and Care NHS Trust seeks a Tissue Viability Nurse to lead specialist wound care, assess patients, plan treatment, and support clinical teams across community and inpatient settings in Norfolk.

You will deliver education, maintain records, contribute to pressure ulcer prevention, and collaborate with NHS, community and voluntary providers to embed pathways. Current NMC registration and a full driving licence are required.

• Maintain tissue viability best practice across Norfolk Adults with community, inpatient, dermatology and lymphoedema teams.
• Maintain a patient caseload, delivering and evaluating specialist care and interventions.
• Assess patients' condition, plan and implement treatment, and keep accurate clinical records.
• Provide wound care advice and support for nursing teams to reduce harm, aid healing and avoid admissions.
• Cover other place clinicians during leave, high demand or as service needs require.
• Support the tissue viability education and training plan and audits in community and ward teams.
• Develop relationships with NHS, community and voluntary sector providers to embed pathways and integrated delivery.
• Work with acute trust tissue viability teams and community and inpatient teams on discharge of patients with complex wounds.
