Job Description
  • Providing practical and emotional support (telephone calls, welfare checks, shopping etc) and guidance to service users
  • Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Responding to referrals from Clinicians and Community Health Professionals
  • Assessing service users' needs, completing and following a support plan for the individual.
  • Supporting service users in the weeks after hospital discharge or to prevent readmission.
  • Taking someone to a hospital appointment they otherwise couldn't attend or picking up prescriptions and shopping.
The Successful Applicant
  • Have a full UK manual driving license.
  • Have a good knowledge of services provided by the NHS and Social Care.
  • Be able to make things great. You know how to improve service quality for the benefit of users.
  • Be professional. You can deal with queries in a diplomatic and confidential manner.
  • Love being flexible. Working hours outside the norm suit you.
