About Our Client

A well-known charity in the Luton area.

Job Description

  • Providing practical and emotional support (telephone calls, welfare checks, shopping, etc.) and guidance to service users.
  • Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Responding to referrals from Clinicians and Community Health Professionals.
  • Assessing service users' needs, completing and following a support plan for the individual.
  • Supporting service users in the weeks after hospital discharge or to prevent readmission.
  • Taking someone to a hospital appointment that they otherwise couldn't get to or picking up prescriptions and shopping.

The Successful Applicant

  • Must have a full UK manual driving licence.
  • Must have a good knowledge of services provided by the NHS and Social Care.
  • Must be able to improve service quality for the benefit of users.
  • Must be professional and able to deal with queries in a diplomatic and confidential manner.
  • Must be flexible, as working hours outside the norm may be required.
