Job summary

St Margaret’s Hospice in Taunton/Yeovil is seeking a Clinical Nurse Specialist for our Advice Line. You will provide telemedicine-supported palliative care, triage referrals, and offer holistic care to patients and families in Somerset.

The role requires a Registered Nurse with specialist post‑graduate palliative care or oncology experience, excellent communication, and a driving licence for occasional home visits. Part-time 22.5 hours/week with shift patterns and benefits are included.

Qualifications

Be a Registered Nurse.
Specialist post‑graduate experience and knowledge in palliative care, acute oncology or chronic condition management.
Remain calm under pressure during busy shifts.
Work effectively with other healthcare professionals to ensure comprehensive assessment and care planning.
Demonstrate excellent communication skills.
Hold a full driving licence to enable travel for occasional home visits.

Responsibilities

Working with a team to respond to advice line queries, referrals and take appropriate action.
Triaging new referrals, signposting appropriately based on the individual needs, and liaising with other services, internal and external.
Supporting and advising patients, using specialist knowledge and skills in physical symptom control, social, emotional and spiritual care.
Assess the appropriateness and urgency of referrals seeking further information and advice as appropriate managing expectation and giving clarity to outcome.
Conducting home visits as required and feeding back to the community CNS team.
