Qualifications

  • Post Graduate Specialist Practitioner Degree is required.
  • Evidence of ongoing education and professional development.
  • Mentorship and assessment training experience.

Responsibilities

  • Lead and manage the Community Resources Team to ensure safe, patient-centred care at home.
  • Coordinate and supervise staff to deliver a seamless home-based service.
  • Support development, implementation and coordination of procedures and caseloads.
  • Travel between patients’ homes, clinics and surgeries as needed.

An exciting opportunity has arisen for a Team Manager with the Community Resources Team in Wrexham.

The purpose of this post is to provide support to the Community Resource Team in the development, implementation and co-ordination of procedures and caseloads.

Main duties of the job

The successful candidate will lead and manage the service to ensure a seamless service is provided to enable patients to be managed safely and appropriately in the home environment. You will coordinate and supervise the team to ensure a patient centered service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
