Establishing clear goals and pathways towards greater independence for service users, you will organize regular person-centred reviews, putting each person’s views and journey at the heart of planning and decision-making.

Working closely with local multi-disciplinary teams and community-based agencies, you will provide holistic, wrap-around support. You will maintain high standards of environmental safety and quality, ensuring a welcoming and inclusive atmosphere. Facilitating service user engagement, involvement, and peer support, you will implement robust quality monitoring methods and provide key performance indicators, identifying and planning necessary actions.
