Provide CIC in Colchester is seeking a Band 4 Team Lead to manage the Care Coordination Centre within our NHS site. This fixed-term, full-time role spans 37.5 hours per week (Mon-Fri, shifts 08:00-18:00) with an end date of 14 June 2027. Salary range £28,392–£31,157 per year.

To lead the day-to-day operations of the Care Coordination Centre, ensuring safe, efficient, and high-quality coordination of patient pathways across community, primary care, acute, mental health, and voluntary sector services. The Band 4 Team Lead provides first-line leadership, supervises staff, manages workflow, oversees operational performance, and acts as the central escalation point for complex coordination issues.
Main duties of the job

Deliver supervision, coaching, and competency development for Band 2–3 staff.
Conduct 1:1s, support appraisals, and contribute to performance management.
Support induction and training of new staff, including systems and SOPs.
Act as the escalation point for complex cases requiring multi-agency coordination.
Ensure CCC staff follow SOPs, failsafe pathways, and information governance requirements
Oversee accurate and timely data entry across CCC systems.
