Job summary

The Forward Trust is seeking a Health & Wellbeing Practitioner to support adult and young men within the Criminal Justice System, helping them improve wellbeing and reintegrate into their communities in Essex. Supporting People on Probation Navigator

Location: Essex

Salary : £26,000 per annum

Vacancy Type: Permanent, Full Time

We're looking for a Health & Wellbeing Practitioner to join our dedicated team, supporting adult and young men within the Criminal Justice System to improve their wellbeing, develop resilience and successfully reintegrate into their communities.

What you'll be doing

Working alongside experienced practitioners and partner organisations, you'll deliver personalised health and wellbeing support that empowers people to make meaningful, sustainable changes.

You'll provide tailored one-to-one and group interventions covering:

  • Family and significant relationships
  • Lifestyle and positive social networks
  • Emotional wellbeing
  • Social inclusion and community integration

From supporting individuals in custody to helping them transition successfully back into the community, you'll play a vital role in every stage of their journey.

You'll be responsible for

What we're looking for

You’ll be someone who is:

  • Passionate about supporting vulnerable people to achieve positive change
  • A skilled communicator who can build trust with people from diverse backgrounds
  • Organised, resilient and confident managing a varied caseload
  • Comfortable delivering both individual and group interventions
  • A collaborative team player who enjoys working with partner agencies

Experience within criminal justice, health, wellbeing, substance misuse, mental health, housing, probation or social care is desirable, but above all we're looking for someone who genuinely wants to make a difference.

All probation-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 6- 16 weeks to complete. Any offer of employment will be conditional upon the successful completion of both checks.
