Job summary

The Forward Trust is recruiting a Health and Wellbeing Practitioner to support prisoners at Usk and Prescoed. You’ll manage a caseload, deliver harm reduction guidance, and work toward personalised care plans while helping individuals prepare for community transition.

You will run 1:1 and group sessions, coordinate through-care, and maintain high-quality records to demonstrate impact. The role requires compassionate, proactive engagement and strong collaboration with partners.

Qualifications

  • Ability to manage a caseload and prioritise needs.
  • Ability to provide harm reduction advice and support.
  • Strong communication and interpersonal skills.
  • Comfort with both 1:1 and group-based interventions.
  • Ability to develop person-centred care plans and track progress.

Responsibilities

  • Manage your own caseload, building positive and supportive relationships with service users.

  • Support individuals with release planning, helping them prepare for life beyond prison.
  • Coordinate through-care and transitional support, ensuring individuals are connected with the right services when returning to the community.
  • Maintain accurate, timely and high-quality records, ensuring data is captured effectively.
  • Work towards agreed targets and outcomes, using data to demonstrate the impact and effectiveness of your work.

Health and Wellbeing Practitioner - HMPs Usk and Prescoed

Are you passionate about helping people make positive changes in their lives? As a Health & Wellbeing Practitioner, you’ll play a key role in supporting individuals in prison to improve their health and wellbeing, reduce harm and prepare for a successful transition back into the community.

What You’ll Be Doing

Please note this is a full time 37.5 hours weekly vacancy working Monday to Friday with one late evening worked a week and one Saturday a month. The role is based across HMP Usk and HMP Prescoed. Both sites are inaccessible by public transport so a car is required.
