About the Role:

Your focus will be on helping individuals progress towards greater independence, ensuring sustainable move-on within defined timescales.

Connect Wiltshire is a multi-agency partnership led by Turning Point and commissioned by Public Health. What you’ll be doing:
  • Manage referrals into the service, ensuring a smooth and supportive assessment process.
  • Create and deliver personalised support plans and risk assessments to help clients thrive.
  • Champion safeguarding by following policies and procedures to keep everyone safe.
  • Empower clients by offering advice, information, and support to connect them with the right services.
  • Oversee the day-to-day running of the service, making sure everything’s safe, running smoothly, and in line with health and safety standards.
What we’re looking for:
  • Experience working with people who may have complex needs and/or challenging behaviours.
  • Able to build effective relationships with clients and services.
  • A valid UK driving licence and access to own vehicle is required - business insurance will be required for roles involving travel to work
