Job summary

Community Integrated Care is seeking a dedicated Support Worker in Stockport to assist adults with learning disabilities, autism and mental health needs in daily life.

You will join a caring team, follow individual support plans, and help individuals engage in meaningful activities, outings and routines with empathy. The role includes evenings and night shifts in a demanding but rewarding setting, with training provided and progression opportunities across a 24-hour contract.

Responsibilities

• Support a gentleman with learning disabilities, autism, rectal prolapse and mental health.
• Provide calm, friendly support with appropriate boundaries.
• Follow the support plan and communicate via calendar and methods.
• Enable regular outings and nature walks; assist with travel by bus/train.
• Assist with activities and enrichment outside the home as needed.

Working closely with the Service Leader and Support Worker Team, you will be responsible for supporting adults with learning disabilities, autism and mental health concerns to live their daily lives. Each person we support is unique, so the support you provide will be person-centred and will focus on achieving the best lives possible with them.

Our Shifts:

9pm to 9am with an occasional day shift which is 9am to 9pm

What You Will Be Doing

You will be supporting a gentleman with learning disabilities, autism, rectal prolapse and mental health.

• First and foremost his family is important to him, he loves spending time with them and also knowing when I will see them.
• He enjoys all kinds of activities and will try anything once.
• He loves travelling to places both new and familiar and being given the choice to do this. He will travel by bus and train - he have a bus and a train pass, so it is important staff use these.
• He likes his staff to be calm, friendly, engaging, and to be able to be firm and have appropriate boundaries with him. He appreciate if staff can do this, and it is an important aspect in managing any challenging behaviour he may have, and more importantly for relationships going forward
• He will want regular reassurance on this, and for staff to follow my support plan and to help communicate this via his calendar/schedule and communication methods. Getting out in nature is very important, so staff should be prepared and fit enough for moderate walks in New Mills or travelling out further to areas of beauty, which is his preference. He will want to get out most days the weather is good - getting out in Nature can turn his day around, if I feel I'm not having the best one.
• He as an activity file in my service that he can choose from/staff can add to if we find a new place or new thing I like to do, so it's important staff are proactive around these things with him.
• He enjoys activities outside the home such as swimming at the gym, so it's important he has support available'

Training will be provided in Administration of medication, Manual handling and Positive Behavioural Support.

We are looking to support a 24 hour Contract.
