Job summary

Responsibilities

• Provide person-centred support to adults, promoting independence, dignity and wellbeing in line with care plans and PBS strategies.
• Assist with daily living activities, including personal care, mobility, nutrition, continence and use of equipment.
• Monitor health needs, attend appointments and maintain accurate records.
• Administer and manage medication safely according to policies and training.
• Work flexibly across days, nights and shifts, including sleep-ins where required.
