Vetro Recruitment is seeking a Support Worker - Supported Housing in Tonypandy to provide social, emotional, and practical support to adults with mental health and trauma-related needs within a housing setting. The role offers a flexible rota including evenings and weekends.

You will assess needs, develop personalised support plans, liaise with external agencies, maintain records, and support independence. NVQ 3 or equivalent, driving licence, Welsh language desirable; access to sleep-in pay

Qualificações

• NVQ Level 3 in Health and Social Care or equivalent.
• Experience supporting individuals with mental health issues.
• Full UK driving licence and access to transport.
• Welsh language ability desirable.
• IT skills including word processing and databases.

Responsabilidades

• Conducting assessments and developing personalised support plans tailored to individual needs.
• Supporting service users to access information, advice, and community resources to promote independence.
• Liaising with external agencies and participating in joint case reviews to ensure comprehensive support.
• Maintaining accurate records of support provided, including risk assessments and support reviews.
• Providing ongoing support and mentoring to empower individuals and promote recovery.
