Thames Reach is seeking a dedicated Support Worker to join the Hounslow Reach Team in London. The role focuses on floating support, tenancy sustainment and short-term interventions, helping clients access housing and related services.

You will provide key-work, crisis support and advocacy, developing person-centred plans. The role requires empathy, strong communication and the ability to work both independently and as part of a team.

Qualifications

• Demonstrated empathy and understanding of vulnerable people.
• Excellent communication and listening skills.
• Ability to work independently while staying accountable to management.
• Ability to collaborate effectively with other teams and external partners.

Responsabilités

• Contribute to continuous improvement of service effectiveness, delivering high-quality outcomes.
• Provide key-work, crisis intervention, case recording, face-to-face and group work, advisory and advocacy support.
• Maintain strong relationships with Thames Reach colleagues to coordinate efforts.
• Produce timely, accurate information, reports and analysis.
• Support caseloads with complex needs and help achieve Education, Training & Employment targets.
• Support service user involvement and KPI achievement.
