Qualifications

• This role requires a compassionate, reliable and proactive individual.
• Ability to provide person-centred support in line with care plans.

Responsibilities

• Provide person-centred support to individuals, promoting independence, choice, dignity and wellbeing.
• Deliver tailored support meeting physical, emotional, social and mental health needs.
• Assist with personal care, mobility, nutrition, continence care and usage of equipment.
• Monitor needs, attend appointments, promote healthy lifestyles, and maintain records.
• Administer and manage medication safely in accordance with policies and training.
• Follow PBS strategies and support individuals during distress with least restrictive practices.
• Safeguard individuals and promote rights, dignity and protection from harm.
• Maintain a safe, clean living environment and support daily tasks and health & safety standards.
• Complete documentation including daily notes, incident reports, health records and financial records.
• Support budgeting, personal finances and accessing benefits.
• Collaborate with families, healthcare professionals and external agencies to achieve positive outcomes.
• Adhere to policies, regulatory requirements and quality standards; participate in training and supervision.
